MDT Rush Delivery Driver Independent Contractor Agreement

Thank you for your interest in joining the MDT Team as a driver. All MDT Employee drivers are required to obtain specific documents prior to starting work. The following items are required:

	Current driver’s license
	Vehicle Registration
	Insurance Policy & Insurance Card
	All drivers must be able to read DC, MD & VA maps
	Clean driving record
	Drivers are preferred to have 2-way radios or cell phone
	All drivers must complete all delivery work and turn in the next business day.
